The IBM (USA) Personal Care Connect (PCC) facilitates the remote monitoring of patients. PCC is an open platform that can integrate devices from device vendors and applications from independent software vendors. Although PCC may have features in common with other remote monitoring systems, it differs from them in that it is a standards-based, open platform designed to integrate with devices from device vendors and applications from independent software vendors. One of the motivations for PCC is to create and propagate a working environment of medical devices and applications that results in innovative solutions.

IntraMed (Denmark) develops and markets decision support systems for the healthcare sector in the form of software-based Disease Management Systems. IntraMed’s solutions create more effective and secure communication between treatment providers in the healthcare services to the benefit of chronically ill patients requiring constant supervision and medication. CareLink Monitoring System
Medtronic (USA) offers a FDA-approved integrated diabetes management system consisting of an internet-based insulin pump, continuous glucose monitoring (CGM) and therapy management software. CareLink Pro is part of the fully integrated CareLink platform that simplifies diabetes data management for patients and caregivers. Patients first upload data from their diabetes management devices to a secure online database using CareLink Personal, an easy-to-use web-based system. Data can also be uploaded directly to CareLink Pro in the general practitioners office. With CareLink Pro, a software program installed on the computer, users can gain access to the patients’ data. CareLink Pro reports can be viewed on-screen, printed, or saved to the computer.

Entra Health Systems (USA) provides integrated healthcare technology solutions with the aim to improve communication between patients, clinicians and caregivers.
One of the services for diabetes management is MyGlucoHealth which is a mobile and online diabetes management platform offering MyGlucoHealth Wireless Meter which is an integrated Bluetooth-enabled blood glucose meter that transmits data (via mobile or pc) directly to MyGlucoHealth Portal and to clinicians and other healthcare professionals who are provided with real-time, accurate and direct readings from the patient’s blood glucose meter. MedApps
MedApps (USA) provides a comprehensive health information management platform and suite of tools that enables this connectivity through the cloud to enhance patient care – from chronic disease management to health and wellness programs.
For glucose monitoring they offer HealthPAL™; a small portable device that uses wireless (Bluetooth) or wired connectivity and M2M cellular technology to collect and transmit health readings from compatible retail medical monitors to a user’s electronic health record. HealthPAL is currently FDA-cleared for use with glucose meters, blood pressure monitors, weight scales and pulse oximeters. The data can be accessed for remote monitoring by professional caregivers using enterprise EHRs, or by the end-user or authorised family members through the patient’s personal health record.

Alive Technologies (Australia) develops wireless health monitoring systems for screening, diagnosis and management of chronic diseases. Their Alive Diabetes Management System provides wireless transmission of blood glucose readings to a central web-based database, allowing an accurate diary to be kept. This can be easily reviewed by the patient and his physician, with two way wireless messaging providing insulin schedules, advice and reminders. Expert systems on the server analyse the data and provide immediate feedback to the patient and his doctor.

Accu-Chek® Combo System
Roche Diabetes (USA) provides a comprehensive assortment of glucose meters, insulin pumps and intergrated devices. The Accu-Chek® Combo System combines a glucose meter, bolus calculating software and an insulin pump in a wearable system.
Blood test results are stored in the meter so that they can be accessed and viewed in a way similar to other types of online software. The Combo meter includes bolus calculator.
